Key Responsibilities\n

• Act as a liaison between clients, their families, and outpatient providers to ensure coordinated care and support systems are in place.

• Engage with insurance companies for pre-certification and utilization management to facilitate smooth access to necessary services.

• Participate actively in discharge planning and aftercare strategies, ensuring a seamless transition from inpatient care to the community.

• Provide therapeutic meal support for clients, emphasizing the modeling of healthy eating habits and fostering a positive relationship with food.

• Deliver empathetic and personalized care that acknowledges and respects clients' unique circumstances and intersecting identities.

• Implement evidence-based therapeutic techniques in individual and group therapy sessions, promoting emotional well-being.

• Document client interactions thoroughly in compliance with state regulatory requirements and agency policies.

• Collaborate with interdisciplinary teams to review client progress and formulate adaptive treatment plans.

Qualifications\n

Compensation and Benefits\n
