Work Environment

As a Home Health Physical Therapist, you will provide onsite services in patients' homes, creating a unique and personal therapeutic environment. Key Responsibilities

• Conduct thorough evaluations of patients' physical health to identify specific rehabilitation needs and create individual treatment plans.
• Utilize various therapeutic modalities such as exercises, manual therapy, and assistive technologies to alleviate pain and enhance mobility.
• Document patient progress rigorously, tracking outcomes and adjusting treatment plans as necessary to meet evolving goals.
• Educate patients and their families on rehabilitation techniques and the use of assistive devices to ensure continuity of care outside therapy sessions.
• Collaborate with a multidisciplinary healthcare team to optimize patient care and promote holistic recovery strategies.
• Stay current with industry practices by attending workshops and engaging with professional societies, thereby enhancing your own skills and knowledge.

Patient Population / Client Focus

In your role, you will serve a diverse clientele ranging from children to older adults, each with unique needs and challenges. Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state licensure in Physical Therapy.

Compensation and Benefits
