The salary range for this position is USD 75,000 to 80,000 annually.

As a vital part of the Community Health Clinic, you will collaborate with a cross-disciplinary team of healthcare professionals, ensuring comprehensive care is delivered to our clients. Key Responsibilities\n

• Conduct in-depth evaluations and assessments of children to identify their individual needs and establish tailored support strategies.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), tailored to the unique circumstances of each family.
\n
• Monitor and assess the outcomes of interventions to ensure they effectively promote the child's growth and development.
\n
• Provide essential support and guidance to families, helping them navigate available resources and enhancing their understanding of child development.
\n
• Comply with all federal and state regulations to maintain the highest standards of services provided.
\n
• Utilize a coaching model and Natural Learning Environment Practices to enable families to engage with their children in meaningful learning experiences.
\n
• Establish positive rapport with children and families, nurturing a safe and supportive atmosphere that fosters learning and growth.
\n
• Administer standardized evaluations and assessments as necessary, ensuring thorough documentation and compliance with agency protocols.

Patient Population / Client Focus\n

This role specifically caters to children and families involved in early intervention services. You will work with infants and toddlers who may have developmental delays or disabilities, providing them with tools and strategies to facilitate optimal growth across various domains—physical, cognitive, social/emotional, and communication skills. Your expertise will be instrumental in nurturing these children’s abilities from a critical stage in their development.

Qualifications\n

• Possession of a Master's Degree or Doctorate of Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
