Position Overview

ProActive Physical Therapy is seeking a dedicated and passionate Physical Therapist to join our esteemed team in Port Saint Lucie, Florida. This full-time position primarily focuses on home care, allowing you to provide personalized rehabilitation services directly in our patients' homes. Work Environment

Your primary work environment will be in patients' homes, providing you with the opportunity to create meaningful connections while assessing and addressing their unique needs in a familiar setting. Our full-time schedule operates from Monday to Friday, 8:00 AM to 5:00 PM, with occasional flexibility required to accommodate patient schedules and clinical operations. Key Responsibilities

• Develop and implement individualized treatment plans aimed at improving patients' mobility, strength, and function, ensuring each plan is tailored to the specific needs and goals of the patient.
• Guide and facilitate patients through therapeutic exercises, stretches, and activities that promote their physical well-being and support their pain management efforts.
• Educate patients and their families about proper body mechanics, effective exercise techniques, and provide comprehensive home exercise programs to bolster their recovery.
• Administer modalities such as electrical stimulation, ultrasound, and heat/cold therapy, ensuring safe and effective delivery to enhance treatment outcomes.
• Monitor and document patient progress diligently, tracking improvements in range of motion, strength, and functional capabilities for accurate progress reports and treatment adjustments.
• Ensure treatment areas are prepared and maintained, with all necessary equipment and supplies organized to support seamless therapy sessions.
• Collaborate with an interdisciplinary team, which may include Occupational Therapists and Speech-Language Pathologists, to coordinate integrated care for patients and maximize therapeutic benefits.
• Provide emotional support and encouragement, fostering a positive rehabilitation environment that motivates patients through their recovery journey.
• Uphold ethical and professional standards, maintaining patient confidentiality and respecting the rights and dignity of all individuals under care.

Patient Population / Client Focus

The diverse patient population served at ProActive Physical Therapy includes individuals recovering from surgery, sports injuries, chronic conditions, neurological disorders, and age-related mobility challenges. Your expertise will cater to varying demographics, from pediatric to geriatric patients, requiring a versatile approach tailored to each unique situation. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state license or certification to practice as a Physical Therapist in Florida.
• At least one year of relevant clinical experience in a rehabilitation setting, particularly in home care is preferred.
• Strong communication and interpersonal skills to establish rapport with patients and their families.
• The ability to work both independently and as part of a collaborative health care team.

Preferred Experience

• Experience in a home care or community-based rehabilitation setting is highly desirable, as it equips you to work effectively in patients' residences.
• Familiarity with administering therapeutic modalities and creating comprehensive care plans to address a variety of patient needs.
• Previous experience working with diverse populations, including geriatrics and pediatrics, is a significant advantage.

Compensation and Benefits
