This is a full-time remote position based out of our Hospice Center. Our virtual team atmosphere allows social workers to maintain a flexible schedule while ensuring quality care through telehealth services. You will be supported by a multidisciplinary team, consisting of psychiatrists, psychologists, and allied health professionals, who champion integrated care and well-being for our patients.

Key Responsibilities\n

• Conduct thorough assessments and evaluations using tools such as the Diagnostic and Statistical Manual of Mental Disorders (DSM), establishing an individualized plan of care that reflects cultural diversity and special needs within our patient population.

• Monitor and document patient progress, providing interventions and strategies that support their mental health diagnoses effectively.

• Prepare and maintain accurate clinical documentation, including treatment records and reports, adhering to regulatory standards and department protocols.

• Collaborate closely with patients and family members to develop comprehensive treatment plans that target specific goals and outcomes while prioritizing patient safety and symptom management.

• Coordinate referrals and facilitate connections between patients and external treatment providers or resources, fostering effective working relationships with other clinicians and agencies.

Compensation and Benefits\n

This full-time position offers a competitive salary in the range of USD 90,000 to 105,000 annually, commensurate with experience.
