Work Environment

This position offers a remote work setting, allowing for flexibility while providing crucial services from a Hospice Center. You will be part of a multidisciplinary team that collaborates closely to ensure holistic care and support for our clients. Working Monday through Friday from 9:00 AM to 5:00 PM, you will have the autonomy to manage your schedule, engage with clients virtually, and contribute to their care plans effectively.

Key Responsibilities

As a Licensed Clinical Social Worker with Lifescapes Counseling, LLC, your responsibilities will include:

• Conducting comprehensive assessments of clients’ psychological and emotional needs, collaborating with medical staff to develop tailored care plans that reflect individual goals and values.
• Implementing effective interventions to manage challenging behaviors and improve coping strategies, ensuring that clients feel supported and understood.
• Providing both individual and group therapy sessions, employing evidence-based practices to facilitate discussions that promote healing and resilience.
• Educating family members about care processes and supporting them in their role, fostering an environment of teamwork and open communication.
• Maintaining meticulous clinical documentation in compliance with regulatory standards, utilizing electronic health record systems to track client progress and interventions.

Patient Population / Client Focus

This role focuses primarily on individuals receiving hospice care—including patients with terminal illnesses and their families. You will work closely with clients facing emotional distress, grief, and complex family dynamics, providing crucial support that empowers them through difficult transitions. Your expertise will be invaluable in enhancing the emotional well-being of those in distress and ensuring the delivery of holistic care that respects the wishes and dignity of every patient.

Qualifications

Candidates for this position must meet the following qualifications:

• A Master's degree in Social Work or a related field.
• Active licensure as a Clinical Social Worker (LCSW) in the state of California.
• A minimum of two years of clinical experience, particularly in mental health or hospice settings.
• Demonstrated proficiency in organizational and communication skills, allowing for effective collaboration within a multidisciplinary team.

Preferred Experience

Compensation and Benefits
