Key Responsibilities\n

• Utilize your education and training to conduct comprehensive psychosocial assessments and formulate independent diagnoses for patients facing a range of mental health challenges.

• Collaborate with clients in the creation of individualized, patient-centered treatment plans that reflect their specific needs, goals, and preferences.

• Employ a variety of evidence-based intervention methods, such as solution-focused therapy, motivational interviewing, and behavioral activation, to facilitate behavioral changes effectively.

• Coordinate care with multidisciplinary teams including other clinicians and primary care providers to ensure a holistic approach that addresses the multifaceted needs of patients.

• Track and document treatment responses meticulously, maintaining oversight of patient progress through virtual check-ins and follow-ups.

• Support psychotropic medication management by monitoring treatment adherence and side effects, ensuring that clients remain informed and involved in their treatment process.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker with Centered Counseling Services, you will work with a diverse client population, including individuals facing anxiety, depression, trauma, and other mental health issues. Your focus will be on providing compassionate, culturally competent care tailored to each client’s unique circumstances and requirements. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• A current, valid LCSW license to practice independently in the State of New York.

Compensation and Benefits\n
