Position Overview

Health Coast Behavioral Center is seeking a Licensed Physical Health Therapist to join our dedicated team in Ontario, California. Key Responsibilities

• Conduct thorough assessments to evaluate patients' physical health needs and develop individualized treatment plans aimed at improving their mobility and overall physical health.
• Utilize evidence-based techniques to deliver physical therapy interventions, including but not limited to therapeutic exercises, manual therapy, and modalities such as heat, cold, and electrical stimulation.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes, documenting all sessions clearly and accurately in compliance with legal and ethical standards.
• Provide education to patients and their families regarding physical health issues, self-care strategies, and ongoing management of health conditions.
• Collaborate with a multidisciplinary team, including medical professionals and mental health therapists, to ensure comprehensive care that addresses both physical and psychological well-being.
• Participate in case conferences, contribute to team discussions, and engage in ongoing professional development activities to stay updated on the latest advancements in physical therapy.

Patient Population / Client Focus

As a Licensed Physical Health Therapist at Health Coast Behavioral Center, you will work with a diverse patient population, encompassing individuals recovering from surgery, those with chronic pain conditions, and patients with various physical disabilities. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Current licensure as a Physical Therapist in the state of California.

• In-depth knowledge of physical therapy practices and a commitment to evidence-based care.
• Demonstrated ability to perform comprehensive evaluations and develop effective treatment plans tailored to individual patient needs.

Preferred Experience

• Experience working in a home care setting or with patients who have mental health concerns is highly desirable.
• Proficiency with electronic health record (EHR) systems for documentation and patient management.
