What You'll Do:

• Screen and evaluate patients with cognitive and ADL disorders
• Develop and implement personalized care plans following clinical standards
• Administer treatments including ADL retraining and therapeutic exercises
• Use standardized assessments to support evidence-based interventions
• Monitor and document patient progress and modify plans accordingly
• Provide counseling and education to patients, families, and caregivers
• Participate in care conferences and team meetings
• Supervise occupational therapist assistants, aides, and students as needed

What You Need:

• Valid occupational therapy license (state-specific)
• Relevant experience in LTC preferred
• Strong communication and teamwork skills
• Ability to follow regulatory and clinical practice standards
