Work Environment

This position operates primarily through a remote setup, allowing for flexibility while serving clients from the comfort of their homes or community settings. Work hours are generally Monday to Friday, from 8:00 AM to 4:30 PM, while occasional evening and weekend hours may be necessary to accommodate clients' needs. On-call availability may be required to address urgent situations or provide support during off-hours. The role demands adaptability to effectively manage client schedules and ensure continuous care.

Key Responsibilities

• Conduct extensive psychosocial assessments to identify the unique emotional and social needs of senior clients.
• Collaborate with clients and their families to craft personalized care plans that align with their individual goals and preferences.
• Provide compassionate counseling and emotional support, assisting seniors in navigating life transitions and the challenges that arise with aging.
• Advocate for clients' rights and help them access vital community resources and services tailored to their needs.
• Work collaboratively with an interdisciplinary team comprising nurses, caregivers, and healthcare professionals to streamline and enhance the overall care experience.
• Perform regular visits to monitor clients' well-being, assess their living conditions, and proactively address any safety concerns.
• Foster open communication and mediate conflicts among family members involved in the care of the senior clients.
• Educate clients and their families about available community resources, support groups, and programs designed to increase social engagement.
• Maintain up-to-date and accurate documentation of all client interactions, assessments, and care plans in compliance with healthcare regulations.
• Stay current with social work practices and policies to ensure adherence to ethical standards in all interactions.

Qualifications

• A Master’s degree in Social Work from an accredited institution.

• Strong knowledge of social work principles related to aging and the availability of community resources.
• Exceptional skills in assessment, counseling, and problem-solving as they pertain to client interactions.

Compensation and Benefits
