This position is 100% remote, providing you with the flexibility to conduct sessions from the comfort of your own home. Key Responsibilities\n

• Conduct comprehensive evaluations for ongoing patient care, ensuring that individual treatment plans are tailored to meet the unique needs of each client.

• Collaborate effectively with interdisciplinary teams, including supervisors, support staff, and family members, to communicate progress and strategize care adjustments.

• Develop a structured plan to outline appointments, home visits, and observation sessions in coordination with supervisory staff to monitor ongoing job performance.

• Organize and conduct home visits for in-depth assessments, including observation of clients' environments and reviews of relevant documentation.
\n
• Administer treatment plans meticulously to ensure compliance with program strategies that enhance patient outcomes.
\n
• Participate actively in team meetings and contribute to the development of new care initiatives or service improvements.
\n
• Deliver monthly and quarterly progress notes to document patient development and outcomes accurately.
\n
• Facilitate training sessions for colleagues to enhance their understanding and execution of the mental health programs.

• Conduct follow-up interviews to assess ongoing patient needs, ensuring a continuum of care.

• Maintain thorough documentation and records to support treatment protocols and regulatory compliance.

• Develop community outreach methods to improve program visibility and facilitate client engagement.

Patient Population / Client Focus\n

The counselor will engage with diverse populations that include adults and youth, dealing with various mental health issues, including anxiety, depression, trauma, and substance use disorders. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• A valid and unrestricted Professional Counselor license (LPC) in the state of Texas is mandatory.

Compensation and Benefits\n
