Fairport Rehab & Nursing Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our interdisciplinary team in providing high-quality geriatric care to our residents. About the Role:

As a Licensed Practical Nurse (LPN) at Fairport Rehab & Nursing Center, you will be responsible for providing direct patient care to our residents, including:

• Administering medications and treatments as ordered by physicians

• Assisting with personal care and hygiene needs

• Monitoring vital signs and reporting changes to healthcare team

• Participating in care planning and development of individualized care plans

• Providing emotional support and education to residents and their families

• Collaborating with other healthcare professionals to ensure seamless delivery of care

Responsibilities:

• Provide skilled nursing care to residents in accordance with established standards and policies

• Develop and implement individualized care plans that address the unique needs of each resident

• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

• Maintain accurate and detailed records of patient care and treatment

• Communicate effectively with healthcare team members and families to ensure a collaborative approach to care

• Demonstrate a strong commitment to patient-centered care and a passion for working with geriatric populations

    Compensation and Schedule:

    • Pay Range: $30.00 – $34.00 per hour with pay commensurate with certification and years of post-License experience
    • Shift Differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts
    • Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm), Night (11:00pm-7:00AM)
    • Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week)
    • Weekend Availability: Required
    • Sign-On Bonus: $3000 Sign-On Bonus for Full-Time LPN's
