Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify patients' mental health status, strengths, and challenges, leading to tailored intervention strategies.

• Provide evidence-based therapies such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to guide patients toward healing.

• Collaborate closely with a multidisciplinary team of psychiatrists, nurses, and other health professionals to coordinate care plans that align with patients' needs.

• Educate patients and their families on mental health diagnoses, treatment options, and the purpose of medications, ensuring an informed care journey.

• Facilitate individual, group, and family therapy sessions using a toolbox of evidence-based treatment modalities.

• Develop and implement individualized treatment plans in partnership with patients, integrating their personal goals and preferences into their care regimen.

• Serve as an advocate for patients navigating the complex mental health system, focusing on their rights and unique needs.

• Lead therapeutic groups and psychoeducation sessions, and participate in discharge planning meetings to ensure smooth transitions for patients.

This position primarily caters to patients experiencing serious illnesses and their families, who often face emotional and psychological distress associated with their health challenges. Qualifications\n

• A Master's degree in Social Work from an accredited program is required for this role.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

Compensation and Benefits\n
