Key Responsibilities\n

• Provide individual and family counseling, therapy, and psychotherapy tailored to client needs within the hospice setting, based on comprehensive assessments.

• Develop, implement, and evaluate personalized treatment plans and discharge plans in collaboration with healthcare team members.

• Conduct in-depth assessments through patient interviews, case history reviews, and observational techniques to formulate appropriate therapeutic strategies.

• Maintain a manageable caseload while providing both individual and collateral therapy to clients identified in your care.

• Review client case files regularly to ensure compliance with documentation standards, verifying that all required assessments, progress notes, and care plans are completed accurately and in a timely manner.

• Adapt and refine assessment protocols in alignment with best practices and regulatory requirements to enhance the therapeutic process.

In this role, you will primarily serve patients in hospice care—individuals facing terminal illnesses and their families who require emotional support, counseling, and practical guidance to navigate complex healthcare decisions. Qualifications\n

• A Master's Degree in Social Work from an accredited institution.
