Key Responsibilities\n

• Conduct thorough mental health assessments via virtual consultations for hospice clients, focusing on personal histories, emotional concerns, and spirituality.

• Develop and implement individualized treatment plans with a focus on enhancing the quality of life for clients and providing support for their families.

• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and grief counseling to address a range of emotional and psychological issues.

• Facilitate both individual and group therapy sessions, fostering personal growth and coping strategies for clients and their families, particularly in managing grief, loss, and end-of-life transitions.

• Coordinate with multidisciplinary team members to ensure holistic and empathetic care tailored to the client’s evolving needs.

• Provide immediate crisis intervention and support when necessary, utilizing effective de-escalation techniques during urgent situations.

• Educate clients and families about mental health resources, coping mechanisms, and community support services, empowering them through knowledge.

• Maintain compliant documentation of clinical interactions, treatment plans, and progress notes, adhering to legal and ethical standards.

• Participate in team meetings, case management discussions, and ongoing training to enhance clinical practices and foster team collaboration.

Patient Population / Client Focus\n

Your primary focus in this role will encompass a diverse array of clientele including individuals diagnosed with terminal illnesses, their families, and caregivers. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LICSW licensure in the state of Washington.

• A minimum of two years of post-licensure experience, preferably in hospice, palliative care, or related mental health settings.

Compensation and Benefits\n

The salary for this position ranges between USD 100,000 and USD 120,000 annually, commensurate with experience and qualifications.
