Key Responsibilities\n

• Conduct thorough psychosocial assessments through detailed interviews, observations, and a comprehensive review of medical records to identify emotional, social, and environmental factors affecting patients’ recovery and quality of life.

• Deliver compassionate counseling and support to patients and their families, addressing issues related to adjustment, grief, and emotional wellness, utilizing evidence-based practices.

• Collaborate closely with the multidisciplinary rehabilitation team to develop and implement individualized treatment plans that incorporate patients' psychosocial needs, ensuring a holistic approach to care.

• Facilitate access to community resources such as support groups and vocational rehabilitation services, effectively connecting patients and families with essential support networks.

• Document all assessments and interventions meticulously in the electronic health record (EHR) system to ensure seamless communication within the team and compliance with regulatory standards.

You will primarily work with patients in hospice care, who are often facing end-of-life challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

Compensation and Benefits\n
