This position is designed for flexibility and work-life balance. You will primarily work from home, providing you with the ability to manage your schedule effectively. While the work is predominantly remote, there may be times when you are required to conduct in-person assessments or client meetings at the hospice center to foster personal connections and provide tailored care. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify and analyze clients' mental health concerns, strengths, weaknesses, and available resources.

• Deliver individualized, family, and group therapy sessions employing evidence-based treatment modalities, including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed approaches.

• Design and implement personalized treatment plans with clear goals and interventions, adapting to each client’s unique needs and feedback.
\n
• Collaborate with psychiatrists, psychologists, and multidisciplinary team members to create and execute coordinated treatment plans, ensuring holistic patient care.

• Facilitate crisis interventions, conduct risk assessments, and engage in safety planning as necessary to support clients during critical moments.

• Document client interactions and treatment progress accurately and promptly in compliance with ethical and legal standards.

• Lead engaging group therapy sessions, psychoeducation classes, and support groups aimed at community building and skill development.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.
