Key Responsibilities\n

• Assist patients in navigating and accessing a variety of community resources, including support groups, vocational rehabilitation services, and housing assistance programs.

• Educate patients and their families about available community resources and guide them in post-rehabilitation planning.

• Engage in discharge planning processes, ensuring smooth transitions to home or alternative care environments, and continuing support as needed.
