Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to guide the development of individualized treatment plans and appropriate discharge plans, ensuring they align with the needs and goals of our clients.

• Collaborate closely with a multidisciplinary healthcare team, engaging in regular discussions about ongoing patient care needs and treatment progress, with a focus on achieving optimal health outcomes.

• Establish and foster meaningful relationships with community organizations and resource groups, representing the clinic in meetings and committees that aim to address the broader social determinants of mental health for patients.

• Provide crisis intervention services and therapeutic counseling for clients experiencing emotional and behavioral challenges, employing a range of therapeutic modalities tailored to each individual's circumstances.

• Maintain consistent and accurate records, including clinical notes and progress documentation, in accordance with departmental policies and regulatory standards for quality assurance.

Qualifications\n
