Key Responsibilities\n

• Conduct initial assessments to understand clients' goals, preferences, and available treatment options, developing tailored rehabilitation plans to suit their individual circumstances.

• Create, implement, and periodically modify treatment plans based on each client's unique educational background, capabilities, and future aspirations.

• Collaborate closely with a range of professionals, including physicians, occupational therapists, and psychologists, to formulate comprehensive rehabilitation programs customized for each client's specific needs.

• Maintain meticulous case files and documentation that include client personal information, eligibility criteria, insurance details, services provided, and client contact narratives, ensuring compliance with legal and ethical standards.

• Monitor client progress continuously, assessing goal achievement and adjusting treatment plans accordingly for optimal results.

• Stay engaged with clients during job training, occupational placements, and other relevant on-site activities, providing guidance and resolving any challenges that arise.

• Work collaboratively with clients' families to identify ways to enhance support systems conducive to meaningful recovery and personal growth.

• Prepare timely reports on clients' needs and progress, ensuring a clear framework for further recommendations and referrals to external resources as necessary.

Patient Population / Client Focus\n

The Telehealth Licensed Professional Counselor will work primarily with clients experiencing mental health and mood disorders, requiring a sensitive and personalized approach. Qualifications\n
