Key Responsibilities\n

• Conduct in-depth psychosocial assessments by interviewing patients, observing interactions, and reviewing relevant medical records to identify emotional, social, and environmental factors that may influence recovery.

• Provide compassionate counseling and support to patients and their families, helping them navigate adjustment issues and emotional challenges related to health changes.

• Collaborate closely with the multidisciplinary rehabilitation team to formulate individualized treatment plans that prioritize psychosocial needs alongside medical ones.

• Assist patients in connecting with essential community resources, such as support groups, vocational rehabilitation services, and housing assistance to facilitate their recovery.

• Educate patients and families about available community resources and post-rehabilitation services, ensuring they are well-prepared for life after care.

• Engage actively in discharge planning processes, ensuring smooth transitions for patients returning home or to other care environments, prioritizing their ongoing support and well-being.

Qualifications\n
