Key Responsibilities\n

• Conduct thorough and empathetic client assessments to understand individual needs and goals, ensuring that you develop personalized care plans.

• Collaborate with interdisciplinary teams to formulate integrated support plans that address all aspects of a patient’s care, from medical to emotional support.

• Provide clinical interventions, such as individual and family counseling, crisis intervention, and resource coordination, aimed at improving patient outcomes.

• Document care assessments, progress notes, and treatment plans accurately and in a timely manner to maintain regulatory compliance and effectively communicate within the team.

Patient Population / Client Focus\n

In this role, you will primarily serve clients within a hospice setting, providing compassionate care to terminally ill patients and their families. You will focus on addressing not only psychological and emotional concerns but also facilitating discussions around end-of-life issues, bereavement support, and advance care planning. Your work will help alleviate suffering and provide necessary support during one of the most challenging times in patients’ lives.

Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited institution.
