Key Responsibilities\n

• Collaborate with the interdisciplinary rehabilitation team to develop and implement individualized treatment plans that reflect the psychosocial needs and preferences of each patient.

• Assist patients in navigating community resources, such as support groups, vocational rehabilitation services, and housing assistance programs, empowering them during their recovery process.

• Educate families about the available community resources and post-rehabilitation services to ensure a smooth transition and sustained support post-discharge.

• Engage in thorough discharge planning and facilitate seamless transitions to home care or alternative settings, ensuring continued patient support.

Compensation and Benefits\n
