- Design and implement comprehensive treatment plans that address a variety of psychiatric and medical needs, ensuring an integrated approach to care. \n
- Facilitate individual and group therapy sessions using evidence-based practices tailored to the unique experiences and challenges faced by each patient. \n
- Collaborate seamlessly with medical providers, promoting coordinated efforts to optimize treatment outcomes. \n
- Provide crisis intervention and immediate support during mental health emergencies, ensuring that patients receive timely care. \n
- Educate patients on the significant links between mental health and physical health, empowering them to actively manage their conditions. \n
- Maintain precise and confidential documentation in compliance with legal, ethical, and organizational standards. \n
- Engage in team meetings, case conferences, and interdisciplinary care planning to ensure continuity and quality of care. \n
- Stay updated with the latest research, best practices, and innovations in integrated mental health care. \n
The therapist will primarily serve patients grappling with co-occurring mental health and medical issues, such as chronic pain, autoimmune disorders, and substance use disorders. This patient demographic often requires a thoughtful, multifaceted approach to treatment, as their psychological and physical health are intricately linked. Your expertise will be pivotal in guiding them towards holistic recovery and improved quality of life.
Qualifications\n- \n
- A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution. \n
- Possession of a current and valid license as an LMFT, LCP, or LCSW in the state of Tennessee. \n
- Demonstrated expertise in addressing the mental health needs of individuals with co-occurring medical conditions. \n
- Proven ability to deliver integrated care and effectively collaborate with medical professionals. \n
- Excellent interpersonal and communication skills, capable of fostering therapeutic relationships. \n
- Strong understanding of professional boundaries, ethical guidelines, and patient confidentiality standards. \n
- Familiarity with electronic health record (EHR) systems and related technology. \n
- A collaborative spirit with a strong commitment to working within a multidisciplinary team environment. \n

- At least 2 years of experience in a mental health counseling role, particularly in an integrated care setting. \n
- Experience with crisis intervention and management. \n
