This role is entirely remote, allowing for flexibility and work-life balance while actively engaging with your clients in a virtual setting. As part of a hospice center, you will be surrounded by a multidisciplinary team that includes nurses, psychologists, and case managers working together to provide holistic care. You will effectively utilize telehealth technology and electronic medical record (EMR) systems to ensure seamless communication and effective documentation of patient progress.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' needs, strengths, and challenges as part of a thorough intake process.

• Develop and implement personalized treatment plans that align with clients' needs and objectives, utilizing evidence-based practices.

• Provide therapeutic services including individual therapy, family counseling, and group therapy via virtual platforms, ensuring personalized care.
\n
• Apply a variety of evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care to enhance clients’ mental health and coping skills.

• Manage crisis situations through immediate intervention techniques and crisis counseling, connecting clients with necessary community resources.

• Collaborate closely with a multidisciplinary team to design integrated care strategies and ensure a cohesive approach to patient treatment.

• Maintain accurate and timely records of patient interactions and treatment progress in compliance with regulatory standards and organizational policies.

• Engage in regular case reviews and team meetings to share insights, strategies, and support ongoing professional growth.

Patient Population / Client Focus\n

In this role, you will work with a diverse population, including individuals with chronic illnesses, mental health conditions, and those at end-of-life. You will provide essential support to families dealing with grief, loss, and complex emotional challenges, ensuring that the care received is empathetic and tailored to their unique needs.

Qualifications\n

• Active LCSW license in good standing in the state of Tennessee.

• Bachelor's degree in social work from an accredited institution is required; however, a Master’s degree in Social Work (MSW) is preferred.

• A minimum of 2 years of clinical experience in outpatient or mental health settings.

• Proficiency in applying evidence-based therapeutic interventions.

• Excellent communication, organizational, and interpersonal skills to foster therapeutic relationships.
