Work Environment

Key Responsibilities

• Conduct comprehensive physical therapy evaluations for pediatric and adult clients, assessing their physical abilities and limitations.
• Design and implement individualized treatment plans based on evidence-based practice to address the specific needs and goals of each patient.
• Regularly reassess patient progress by monitoring clinical signs and symptoms and adjusting treatment plans as needed to ensure effective recovery.
• Utilize Heal Motion Care's electronic record system to complete all required documentation, including SOAP notes and progress summaries, in a timely manner.
• Provide education and support to patients and their families regarding the recovery process, addressing potential challenges and setting realistic expectations.
• Achieve established performance metrics related to patient utilization goals and contribute positively to the business objectives of Heal Motion Care.
• Collaborate with interdisciplinary teams, coordinating care for clients receiving multiple therapy services to optimize overall treatment outcomes.
• Promote our organizational values and contribute to a positive team culture, acting as an advocate for Heal Motion Care in the community.

Qualifications

Compensation and Benefits
