Work Environment

Located in the vibrant community of Hialeah, our facility operates onsite within a school, allowing FNPs to directly engage with students and families in a familiar environment. The center promotes a flexible weekly schedule; typically, the work hours are Monday, Wednesday, and Friday from 8:00 a.m. to 5:00 p.m., accommodating the needs of both our staff and the community we serve. Key Responsibilities

• Deliver comprehensive primary care services tailored to the needs of students and their families, including assessments, treatment plans, and follow-ups.
• Conduct onsite health risk assessments to identify and address potential health issues proactively.
• Provide wellness education aimed at promoting preventive measures and encouraging healthy lifestyle choices amongst patients.
• Support patients in implementing positive health changes, improving both acute and chronic conditions through ongoing guidance.
• Collaborate with medical staff at Southern Healthcare Center to ensure high-quality care and effective patient outcomes.
• Utilize telehealth platforms to consult with patients remotely, ensuring access to healthcare regardless of circumstances.

Patient Population / Client Focus

This position focuses on a diverse patient population, including children, adolescents, and their families found in the school system. Qualifications

• Master's Degree in Nursing with a specialization in Family Practice.
• Valid and active Family Nurse Practitioner License in the state of Florida.

• Proficient in utilizing telehealth technology for clinical consultations and follow-ups.

Compensation and Benefits
