Key Responsibilities\n

• Conduct thorough psycho social assessments to ascertain patients' needs, including identifying crises and formulating appropriate responses to crises that arise.

• Develop, implement, and modify comprehensive treatment plans for patients according to their evolving mental health needs, ensuring that all interventions are appropriate and effective.
\n
• Collaborate with a multidisciplinary team, including healthcare providers, caregivers, and community agencies, to devise and execute discharge plans that promote continuity of care.
\n
• Stay compliant with all federal, state, and local regulations, adhering to the Mental Health Practice Act while ensuring that care standards meet organizational quality expectations.
\n
• Assess, organize, and prioritize the daily clinical workload, focusing on new admissions, current patients, and those preparing for discharge.
\n
• Provide psychosocial support and conduct social work consultations for patients and their families, offering crisis intervention and bereavement services as needed.
\n
• Foster effective communication across multidisciplinary teams and external agencies to coordinate care, facilitate timely discharge, and identify and resolve any patient care delays.
\n
• Actively advocate for patients to enhance access to resources and maintain open lines of communication between caregivers, families, and other service providers.
\n
• Identify patients at risk for prolonged stays, readmission, or complex discharge needs and coordinate with team leaders to optimize care solutions.

Patient Population / Client Focus\n

As a Mental Health Counselor, you will work primarily with individuals from diverse backgrounds dealing with a range of mental health disorders, including anxiety, depression, PTSD, and other related conditions. You will provide care that is tailored to each patient's unique circumstances, strengthening their resilience and facilitating recovery through compassionate support and evidence-based interventions.

Qualifications\n

• A Master’s Degree in Social Work, Counseling, or a related field obtained from an accredited institution, with verification of degree required.

• Current state licensure as an LCSW, LMFT, LMHC, or Psychologist.

• Exceptional verbal and written communication skills, allowing for effective client interaction and multidisciplinary collaboration.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from $90,000 to $100,000, commensurate with experience and qualifications. Additionally, you will have the opportunity for an up to $1,000 sign-on bonus, which helps support your initial transition.
