Key Responsibilities\n

• Conduct thorough diagnoses and develop individualized treatment plans for various mental, emotional, and behavioral disorders, making adjustments as needed to reflect changes in patient status.

• Facilitate referrals to additional mental health services, therapy groups, community resources, and support programs to enhance patient well-being.

• Collaborate with an interdisciplinary team to create effective discharge plans, ensuring comprehensive follow-up services and continuity of care for patients.
\n
• Adhere strictly to clinical guidelines as well as state and federal regulations, acting within the legal frameworks outlined in the Mental Health Practice Act.

• Prioritize and organize a daily workload based on an assessment of incoming, active, and discharging patient needs within your caseload.
\n
• Offer clinical consultation to patients and their families, conducting psychosocial assessments, offering crisis support, and other interventions as necessary.
\n
• Engage in proactive communication with various healthcare teams and external agencies to guarantee that patients receive quality treatment, timely discharges, and to efficiently resolve any barriers to service.

• Provide emotional support and debriefing for staff involved in critical incidents or adverse events in collaboration with the Employee Assistance Program.
\n
• Actively advocate for patients and their families to ensure their voices are heard within multidisciplinary care teams, outside organizations, and during service coordination.
\n
• Identify patients at risk of complications such as extended stays or readmissions and devise coordinated plans with providers for effective resolution.

Patient Population / Client Focus\n

The Mental Health Counselor will work primarily with a diverse population that includes children, adolescents, and adults facing various mental health challenges. You will address issues such as anxiety, depression, trauma, and behavioral disorders, working closely with patients to tailor interventions that suit their individual backgrounds and unique experiences.

Qualifications\n

• A Bachelor’s Degree in Social Work or a related mental health discipline from an accredited institution, with a valid state licensure as an LCSW, LMFT, LPC, or psychologist.

Compensation and Benefits\n
