Key Responsibilities\n

• Conduct comprehensive assessments to understand the unique needs of patients and develop personalized treatment plans.

• Effectively communicate patient progress with supervisors, family members, and other caregivers through regular updates and meetings.

• Schedule and conduct follow-up appointments, home visits, and observations to ensure ongoing support and effective treatment.

• Implement the treatment program plans and verify they are carried out effectively across various settings.

• Participate in interdisciplinary team meetings to collaborate on treatment strategies while contributing insights based on your assessments.

• Maintain thorough documentation, including monthly and quarterly progress notes, to track patient advancements.

• Coordinate training for staff on program implementation and documentation to enhance service delivery.

• Act as a liaison with the mental health community to promote developmental goals and facilitate partnerships.

• Assist in developing training materials that elevate the standards of care provided to our clients.

Qualifications\n
