Work Environment\n

This full-time role allows you to work remotely, providing you with flexibility while enabling you to deliver essential services to clients at a Hospice Center. You will be part of an interdisciplinary team that includes healthcare professionals and caregivers, devoted to ensuring that seniors receive the best care possible. The standard working hours are Monday through Friday, from 8:00 AM to 4:30 PM, with some flexibility required for client appointments and potential on-call support for urgent situations.

Key Responsibilities\n

• Conduct in-depth assessments to evaluate the psychosocial needs of senior clients, taking into consideration emotional, mental, and social factors.

• Develop personalized care plans in collaboration with clients and their families, addressing individual goals and preferred approaches to care.

• Provide counseling and emotional support to help seniors navigate significant life transitions, such as health changes, loss, or living situation adjustments.

• Advocate for clients' rights and make informed referrals to community resources that support their well-being.
\n
• Work closely with an interdisciplinary team, including nurses and healthcare professionals, to coordinate comprehensive care that addresses the physical, emotional, and social needs of clients.

• Make regular home visits to monitor clients’ well-being, assess their surroundings, and ensure safety while addressing emerging concerns.

• Facilitate family communications and mediate conflicts when necessary to promote collaborative care among family members.

• Provide education on available community resources and support groups to enhance social connections and overall quality of life for clients.

• Maintain accurate documentation of assessments, care plans, and client interactions in compliance with ethical standards.

• Stay updated with industry best practices in social work to ensure compliance and implement ethical, effective interventions.

This role will specifically focus on serving seniors, many of whom are dealing with chronic illnesses or terminal conditions requiring delicate social support and intervention. Understanding the physical and psychosocial challenges faced by this population is key to fostering effective therapeutic relationships and improving patient outcomes.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Clinical Social Worker (LCSW).

• A minimum of two years of experience in social work, ideally in a community health or geriatric setting.

Preferred Experience\n
