Work Environment

This is an onsite position, focusing primarily on home care within the local Woodbridge community. Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities, developing individualized treatment plans that cater to their specific needs and rehabilitation goals.
• Deliver skilled therapeutic interventions such as exercise programs, manual therapy techniques, and various modalities tailored to geriatric patients.
• Regularly monitor and document patient progress, making necessary adjustments to treatment modalities to ensure optimal recovery outcomes.
• Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, occupational therapists, and speech therapists, to create a holistic care approach for each patient.
• Educate patients and their families on therapeutic techniques, such as home exercise programs and injury prevention strategies, ensuring they know how to maintain their health effectively.
• Implement and oversee fall prevention programs and safety measures in patients' homes, critical for reducing fall risks within the geriatric population.
• Stay current with advancements in physical therapy through continuous education and professional development opportunities, enhancing both clinical skill sets and service quality.

Compensation and Benefits
