Key Responsibilities

• Conduct comprehensive evaluations of patients, incorporating a thorough review of medical history and conducting specific assessments to form a tailored treatment plan.
• Effectively communicate evaluation results and treatment options to patients and their families, ensuring they are actively involved in the goal-setting process.
• Implement individualized treatment protocols based on evaluation findings while closely monitoring patient progress and adjusting care plans as necessary.
• Collaborate with other healthcare professionals to discuss changes in patient conditions and coordinate the appropriate modifications to treatment plans.
• Manage discharge planning, ensuring continuity of care upon transitioning from therapy; accurately document all patient care activities, including evaluation reports and follow-up care plans.
• Maintain effective time management skills to meet productivity expectations while also engaging in local marketing initiatives to foster community relations.
• Participate in departmental planning and goal-setting sessions, adhering to all regulatory and accreditation standards.
• Actively pursue ongoing professional development and continuing education opportunities, serving as a mentor to students and new staff.
• Champion an exemplary patient experience by demonstrating adaptability and flexibility in delivering care.
• Contribute to efforts aimed at removing barriers to patient access, utilizing measurement data to inform service delivery improvements.

Patient Population / Client Focus

In this role, you will work primarily with patients in their homes, ranging from individuals recovering from surgeries to those managing chronic illnesses. You'll have the opportunity to work with a wide demographic, tailoring your approach to meet each client's specific needs. Qualifications

• A Bachelor's degree in Physical Therapy is required, with a Doctorate in Physical Therapy (DPT) preferred.
• Current and valid physical therapist license in the state of Virginia.
• Certification in Basic Life Support (BLS) for Healthcare Providers is mandatory.
• Strong interpersonal skills and an ability to build rapport with clients and their families.

Preferred Experience

• At least two years of experience in physical therapy practice, particularly within home health settings.

Compensation and Benefits
