Work Environment\n

This position offers a full-time remote work setting, providing flexibility while ensuring that our specialists can meet the diverse needs of our clients. The standard work schedule is Monday to Friday from 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client circumstances. On-call availability may also be required to provide urgent support during off-hours. Key Responsibilities\n

The Licensed Clinical Social Worker will be responsible for:

• Conducting comprehensive assessments of clients’ psychosocial needs, taking into account their emotional, mental, and social situations.

• Developing personalized and actionable care plans in collaboration with clients and their families to set and achieve specific objectives.

• Offering ongoing counseling, emotional support, and guidance to seniors, helping them navigate transitions such as aging, loss, or health changes.

• Advocating for clients’ rights and ensuring access to community resources necessary for their care.

• Collaborating seamlessly with interdisciplinary teams, including caregivers, health professionals, and other social workers, to enrich the clients’ care experience.

• Conducting regular site visits to evaluate clients' living conditions and well-being, addressing safety issues as they arise.

• Facilitating family meetings to improve communication, resolve conflicts, and work toward unified care goals for the client.

• Educating clients and families on potential community resources, support groups, and programs tailored to enhancing their social engagement.

• Maintaining accurate and timely documentation of assessments, care plans, and progress notes in compliance with ethical standards and regulatory requirements.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

This position offers a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience.
