Key Responsibilities\n

• Psychosocial Assessments: Conduct timely and thorough assessments of patients, utilizing a robust understanding of psychosocial factors affecting health outcomes and ethical considerations.

• Clinical Interventions: Deliver supportive counseling to patients and their families and facilitate access to community resources as needed. You will assist individuals facing challenging decisions such as hospice arrangements or end-of-life care.

• Triage and Caseload Management: Skillfully prioritize a diverse clinical caseload, addressing high-risk situations and ensuring seamless patient transitions during discharge planning in collaboration with case managers.

• Legal and Ethical Compliance: Ensure all clinical practices adhere to established ethical standards and legal frameworks, addressing issues such as suspected abuse or conservatorship matters as they arise.

• Documentation: Maintain accurate and comprehensive documentation of treatment plans, progress notes, and interdisciplinary care updates in accordance with organizational policies.

• Educational Contributions: Participate in in-service training for hospital staff, offering insights on psychosocial impacts in patient care through workshops and discussions.

• Quality Improvement: Engage in initiatives to monitor and improve patient care practices, actively contributing to the evaluation of care processes and outcomes.

Patient Population / Client Focus\n

You will work primarily with patients experiencing various medical challenges, including cancer care, chronic illnesses, and mental health issues. Given the complexity of these conditions, your clinical judgment and compassion will be essential in aiding patients and families as they face difficult transitions and decisions related to health management.

Qualifications\n

• A Master's degree in Social Work from an accredited graduate school.

• Minimum of 2 years of experience in a medical setting, with at least 1 year in oncology or cancer-focused care.

• Current Licensure as a Clinical Social Worker (LCSW) in California.
\n
• Basic Life Support (BLS) Certification from the American Heart Association.
\n
• Fire Life Safety Training (LA City) within 30 days of hire, if applicable.
\n
• Excellent communication skills in English, both verbal and written.
\n
• Strong analytical, problem-solving, planning, and evaluation skills.
\n
• Demonstrated ability to work both independently and collaboratively within a multidisciplinary team.

Preferred Experience\n

Compensation and Benefits\n

The annual base salary for this position ranges from USD 120,000 to 130,000 based on experience and qualifications.
