Work Environment

This position is primarily onsite, as you will travel to clients' homes to deliver physical therapy services. Key Responsibilities

• Conduct thorough evaluations of clients' physical capabilities and needs by utilizing various diagnostic assessments to guide treatment planning.
• Develop and implement individualized therapy plans aimed at improving strength, mobility, and functionality, while preventing disabilities and alleviating pain.
• Supervise and support Physical Therapy Assistants in executing treatment plans and ensure they adhere to best practice guidelines.
• Assess the client's home environment and make tailored recommendations to enhance their independence and safety during rehabilitation.
• Deliver a wide range of therapeutic interventions including manual therapies, mobility training, and functional activity guidance, designed to optimize clients' physical health.
• Document all clinical interactions meticulously, including initial evaluations, progress notes, and comprehensive discharge summaries that comply with OASIS requirements.
• Engage in regular interdisciplinary case conferences to evaluate client progress and adapt care plans as necessary based on ongoing assessments.
• Foster a culture of communication by advising clients and their families on treatment goals, progress, and any required adjustments to care plans.
• Ensure timely submission of clinical documentation and administrative orders following branch policies.

Patient Population / Client Focus

The primary focus of this role will be on individuals requiring home-based rehabilitation services, including elderly patients with mobility challenges, recent surgical patients, and individuals managing chronic pain conditions. Your expertise will empower clients to regain independence and improve their quality of life through effective therapeutic strategies.

Qualifications

• A valid physical therapy degree from an accredited institution with current appropriate licensure in Virginia.
• A minimum of one (1) year of experience as a Physical Therapist, ideally with exposure to home care settings.
• Current CPR certification to be maintained throughout employment.
• A strong commitment to quality patient care, emphasizing integrity, and clinical excellence.
• Exceptional interpersonal and communication skills, fostering effective engagement with clients and their families.
