Work Environment\n

This position is completely remote, allowing for flexibility while maintaining close collaboration with your colleagues and team. You will be working in a Hospice Center environment, where your primary focus will be to support families and children with disabilities or developmental delays. The ability to adapt to varying circumstances and foster open communication with team members is essential for success in this role.

Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children enrolled in our hospice programs, ensuring tailored care that aligns with their developmental needs.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP), closely monitoring each child's progress and providing feedback to optimize treatment outcomes.
\n
• Collaborate actively with a multidisciplinary team, sharing insights and strategies that promote positive health outcomes for children and families.
\n
• Utilize evidence-based practices to develop a joint plan with families, recommending strategies that align with Natural Learning Environment Practices.
\n
• Provide ongoing support and education to families, empowering them with information and skills that foster their child's emotional and developmental health.
\n
• Implement the IFSP effectively, recommending service frequency based on continuous assessment of development and family priorities.
\n
• Adhere to all Federal and State regulations, ensuring compliance and maintaining the health and safety standards for both staff and clients.
\n
• Facilitate workshops or sessions for families regarding child development, utilizing coaching models and natural environment teaching methods.
\n

Patient Population / Client Focus\n

This role focuses primarily on children under the age of three who are facing developmental challenges. As an LCSW, you will work closely with families, developing a strong rapport that fosters trust and open communication. Your work will help children achieve crucial developmental milestones in a supportive hospice setting, where emotional and psychological support is equally prioritized.

Qualifications\n

• Master's Degree or Doctorate in Social Work.

• Must be licensed by the Department of Public Health as a Licensed Clinical Social Worker (LCSW).

• A valid driver's license is required to facilitate travel for home visits, if necessary.

• Strong organizational skills and the ability to manage time effectively are essential for navigating multiple cases.

• Excellent communication skills are necessary for establishing collaborative relationships with families and team members.
