Work Environment\n

This position is remote, allowing you the flexibility to work from home while still being an integral part of our interdisciplinary team based at our hospice center. You will interact with fellow healthcare professionals, including nurses, physicians, and occupational therapists, through virtual meetings and collaborative platforms, ensuring seamless communication and coordination of care. Regular business hours are included, with occasional evening or weekend work to meet the needs of patients and their families during critical times.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, patient observations, and review of medical records to evaluate the emotional, social, and environmental factors affecting patients’ well-being.

• Provide one-on-one counseling to patients and their families, helping them cope with adjustment issues, grief, and end-of-life considerations.

• Collaborate with the rehabilitation and clinical team to craft individualized treatment plans that align with patients' psychosocial needs, ensuring that holistic care practices are incorporated into their hospice experience.

• Assist patients in accessing essential community resources, such as support groups, vocational rehabilitation, or housing assistance that may enhance their quality of life.

• Engage in educational activities for patients and families regarding available community support and post-rehabilitation resources to aid in their ongoing care.

• Participate actively in discharge planning, facilitating seamless transitions from hospice care to home or other community settings.

Qualifications\n

The position offers a competitive salary ranging from USD 75,000 to 100,000 annually, commensurate with experience and qualifications.
