Work Environment\n

This position is remote, allowing for flexibility in work hours while collaborating closely with clients and interdisciplinary teams. Our clinic maintains a strong emphasis on teamwork and open communication, allowing social workers to share insights and resources that enhance client care. Full-time team members work Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evenings and weekends when necessary to meet client needs.

Key Responsibilities\n

• Conduct detailed assessments of clients' psychosocial needs, focusing on emotional and mental health aspects and considering social factors that may impact well-being.

• Work directly with clients and their families to create tailored care plans that align with their individual goals, preferences, and challenges.

• Provide counseling and emotional support, guiding clients through significant life transitions and accompanying them on their journey toward better mental health.

• Advocate for clients by ensuring they have access to community resources and support services, safeguarding their rights, and enhancing the quality of their care.

• Collaborate effectively with a multidisciplinary team, including nurses, caregivers, and other healthcare professionals, to coordinate comprehensive care plans and improve client outcomes.
\n
• Regularly visit clients when necessary, monitoring their health, assessing their living conditions, and addressing any immediate concerns or issues.
\n
• Facilitate family communication and mediate conflicts regarding a client’s care, ensuring that all parties are aligned in support of the individual.
\n
• Provide educational resources to clients and families about available community programs and support groups to foster social engagement.
\n
• Maintain meticulous documentation of client interactions, assessments, care plans, and progress notes within our electronic health record (EHR) system, ensuring compliance with relevant regulations and standards.

• Continuously update knowledge of social work best practices, ethical standards, and local community resources.

Patient Population / Client Focus\n

This role primarily serves senior clients, many of whom face complexities related to aging, mental health challenges, and varying degrees of social support. The LCSW will work with clients and their families to navigate these challenges and promote emotional health and resilience.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in social work, ideally in community health settings or geriatric care.
