Key Responsibilities\n

• Collaborate closely with multidisciplinary healthcare teams—including physicians, psychologists, and other therapists—to develop and implement individualized treatment plans, ensuring a comprehensive approach to patient care.

• Conduct psychosocial assessments to inform treatment strategies and discharge planning, utilizing accepted social work interventions to foster viable recovery pathways.

• Engage in ongoing community outreach, establishing and maintaining positive relationships with community resource leaders. Represent Redesign Counseling Hub at meetings and on committees focused on addressing the social needs of our patient population.

• Provide counseling and crisis intervention to clients, utilizing strong clinical judgment to assess and treat emotional and behavioral problems effectively and promptly.

• Document patient progress, needs, and concerns meticulously, maintaining accurate clinical records and progress notes according to departmental and regulatory standards.

Patient Population / Client Focus\n

Our primary focus is on serving a diverse outpatient population that includes children, adolescents, and adults dealing with various mental health challenges. This role involves working with clients facing issues such as anxiety, depression, trauma, and behavioral problems, utilizing tailored interventions and support systems designed to empower them in their recovery journey.

Qualifications\n
