Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observations, and review of medical records to gauge patients' emotional, social, and environmental challenges impacting their recovery.

• Assist patients in identifying and accessing community resources including support groups, vocational training, and housing assistance to facilitate reintegration into their communities.

• Deliver educational materials and workshops to patients and families regarding post-rehabilitation services, community resources, and coping mechanisms.

• Engage in proactive discharge planning, ensuring seamless transitions to home or alternative care settings after rehabilitation.

Patient Population / Client Focus\n

The ideal candidate will engage with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Qualifications\n

• Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).

Compensation and Benefits\n
