Key Responsibilities\n

• Conducting individual and family counseling sessions through telehealth platforms, addressing psychosocial needs and supporting patients in managing their conditions.

• Utilizing electronic medical records (EMR) to meticulously monitor patient progress, document interventions, and communicate effectively regarding treatment goals.

• Collaborating with a multidisciplinary team of health care professionals to ensure a cohesive approach to each patient's care plan, anticipating needs, and adapting support strategies accordingly.

• Providing essential information and resources to patients and their families about transition plans, ensuring they are well-informed about available services.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling.

• Active licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

• Strong organizational skills and the ability to manage a significant caseload effectively, prioritizing patient needs.

• A demonstrated commitment to improving the lives of clients and advancing the field of behavioral health.

• Proficiency in utilizing electronic medical records and telehealth technologies.

• Excellent communication skills, particularly in written digital communication.

Compensation and Benefits\n

We offer a competitive salary range of $100,000 to $105,000 per year, commensurate with experience and caseload.
