As a Home Health Physical Therapist, you’ll provide individualized, one‑on‑one care where patients are most comfortable — at home. You’ll help patients regain mobility, improve strength, reduce pain, and achieve greater independence while enjoying the flexibility and variety that comes with a career in home health. What You’ll Do As a Physical Therapist , you will: Perform comprehensive initial evaluations and identify patient needs, functional limitations, and appropriate therapy goals Develop and implement individualized plans of care designed to improve mobility, strength, balance, and overall function Provide skilled physical therapy interventions in patients’ homes Evaluate patient progress and adjust treatment plans to achieve the best possible outcomes Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and continued progress Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary care team Maintain timely and accurate clinical documentation in accordance with company and regulatory guidelines Follow infection-control and universal-precaution standards Help patients remain safe, independent, and successful in the place they call home What We’re Looking For Graduate of an accredited Physical Therapy program CPR certification Valid driver’s license and reliable transportation Home health experience preferred Homecare Homebase (HCHB) experience is a plus Strong communication, organization, and time-management skills Why Choose Choice Health at Home?
