Work Environment

This role will be conducted on-site in the homes of our patients throughout the Valley Stream area. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and limitations to develop tailored treatment strategies.
• Create and implement individualized treatment plans focusing on enhancing strength, mobility, and functional independence.
• Utilize a variety of therapeutic techniques including manual therapy, therapeutic exercises, and other modalities to address specific physical impairments.
• Continuously monitor and document patient progress, making necessary adjustments to treatment plans based on observed outcomes and patient feedback.
• Engage in regular collaboration with an interdisciplinary care team, integrating insights from other healthcare providers to ensure comprehensive patient care.
• Provide education to patients and their families about home exercise programs and strategies to maintain physical health outside the clinical setting.
• Adhere to all relevant state and federal regulations, and maintain the highest standards of professional practice.

Patient Population / Client Focus

The primary focus of your work will be with the geriatric population, assisting patients recovering from surgeries, managing chronic conditions, or rehabilitating following injuries. Understanding the unique challenges faced by older adults in a home environment is crucial, as you will implement strategies aimed at improving not only physical capabilities but also enhancing their quality of life.

Qualifications

• Must possess a current and valid license to practice as a Physical Therapist in New York State.
• A Bachelor's degree in Physical Therapy is required; however, candidates with a Master's degree or Doctorate in Physical Therapy from an accredited institution are preferred.

Compensation and Benefits
