LPN Night Shift

LPNWe are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Meadow Falls of Conneaut, a premier long-term care facility dedicated to providing exceptional care and services to our residents. As an LPN at Meadow Falls of Conneaut, you will have the opportunity to make a meaningful difference in the lives of our residents and their families. Every other weekend and 12 hr shifts.$1500 sign on BonusResponsibilities:Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activitiesDevelop and implement individualized care plans in collaboration with the interdisciplinary care teamMonitor and report changes in patient condition to the nursing staff and medical teamCollaborate with other healthcare professionals to ensure comprehensive care and supportDocument patient information accurately and thoroughly in accordance with facility policies and proceduresParticipate in ongoing education and training to maintain current knowledge and skillsDemonstrate a commitment to providing excellent customer service and promoting a positive and respectful work environmentRequirements:Current LPN licenseMinimum 0 years of experience as an LPNAbility to work effectively in a fast-paced environmentStrong communication and interpersonal skillsAbility to lift, push, and pull equipment and supplies (up to 50 pounds)At Meadow Falls of Conneaut, we are committed to providing high-quality care and services to our residents and their families.
