Key Responsibilities\n

• Provide clinical services to clients, both in-office and remotely, focusing on those experiencing symptoms of major mental illness and severe emotional disorders.

• Conduct brief, evidence-based screenings and assessments to determine the appropriate level of care and resources needed.

• Establish and maintain positive rapport with clients and collaborate with team members to achieve common goals.
\n
• Create and implement integrated care plans, ensuring access to linked services while facilitating communication among clients, family members, and providers.

• Engage in crisis intervention as needed, offering immediate support and resources to clients in distress.

• Maintain professional ethical standards and adhere to the code of conduct set forth by Bayfront Healthcare Center.

• Carry a caseload of clients, providing individual, group, and family therapy, tailored to the specific needs and requests of each client.

• Monitor client progress, ensuring timely completion of reports and data submissions, as well as compliance with admission and discharge criteria.

• Participate in committees focused on enhancing agency service delivery and continual quality improvement.

This role primarily supports individuals struggling with major mental illnesses and those in need of detoxification from substance use. You will work with diverse populations, including but not limited to, adults from under-served communities, providing culturally competent care that emphasizes choice and empowerment.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution is required.

• Must hold a current and valid LCSW license to practice in the state of New York.

• Strong knowledge of assessment, diagnosis, and treatment practices for assigned populations, including risk assessment and crisis management.

• Excellent verbal and written communication skills, with the ability to convey complex information effectively.

• Demonstrated organizational and planning skills necessary for managing a caseload.

• Familiarity with developmentally appropriate practices and age-related concerns for varied client populations.

Preferred Experience\n

Compensation and Benefits\n
