Work Environment\n

This remote position allows Social Workers to engage with clients directly from their home office while traveling to schools or clients' homes as needed. The typical workweek is Monday to Friday, from 8:00 AM to 4:30 PM, with flexibility required for occasional evening or weekend client visits.

Key Responsibilities\n

• Conduct thorough assessments of seniors' psychosocial needs, analyzing emotional, mental, and social factors to develop individualized care strategies.

• Collaborate closely with clients and their families to create and implement tailored care plans that align with their specific needs and preferences.

• Provide ongoing counseling and emotional support to assist seniors and their families during significant life transitions and challenges, fostering resilience and coping strategies.

• Advocate for client rights by navigating complex systems to ensure access to necessary community resources and services.

• Regularly monitor client progress through scheduled visits, addressing safety concerns and maintaining comprehensive documentation of interactions and outcomes in electronic health records (EHR).

• Facilitate family communications to resolve conflicts and ensure that care remains aligned with the client's best interests.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is mandatory.

Compensation and Benefits\n
