Key Responsibilities

• Conduct thorough patient evaluations, including reviewing medical histories, performing physical assessments, and documenting findings to develop individualized treatment plans.
• Communicate evaluation results and treatment strategies effectively to patients and their families, involving them in the goal-setting process to ensure alignment with patients' health objectives.
• Implement treatment protocols with precision, based on clinical assessments and dynamic patient presentations, while adapting strategies as necessary in response to changes in patients' conditions.
• Collaborate with multidisciplinary healthcare professionals, recognizing when to modify the plan of care or seek additional medical or rehabilitation consultation.
• Manage discharge planning and follow-up processes effectively, providing clear documentation of patient care activities that reflects the patient's progress and care continuum.
• Demonstrate excellent time management skills, maintaining accurate scheduling and fulfilling productivity expectations while balancing administrative duties.
• Engage actively in departmental initiatives and contribute to achieving departmental goals, adhering to applicable regulations and accreditation standards.

Qualifications

Compensation and Benefits
