Work Environment

The majority of your work will be conducted onsite in the homes of patients, providing a unique opportunity to evaluate and enhance their living environments. Key Responsibilities

As a Physical Therapist at Saber Rehabilitation, you will:

• Direct and administer physical therapy treatment plans aimed at preventing disability, relieving pain, and improving functional mobility in patients, in accordance with established care protocols.
• Oversee and provide guidance to Physical Therapy Assistants, ensuring that all prescribed therapies are executed effectively.
• Evaluate patient functional abilities through comprehensive diagnostic assessments that explore muscle, nerve, joint, and overall physical capabilities.
• Assess home environments to make recommendations that foster independence and enhance rehabilitation outcomes for patients.
• Deliver therapeutic services as prescribed by physicians, which may include manual therapeutic exercises and the application of physical agents such as traction and massage.
• Fit and adjust prosthetic and orthotic devices, providing necessary modifications to healthcare providers when required.
• Maintain meticulous clinical documentation, including initial evaluations, care plans, progress notes, and discharge summaries, to ensure accurate patient records.
• Engage in interdisciplinary team meetings to contribute insights for comprehensive treatment planning and to foster collaborative approaches to patient care.
• Educate patients and their families on managing their physical therapy regimens and the proper use of therapeutic devices.
• Conduct regular on-site assessments to evaluate treatment effectiveness and adjust care plans as needed.
• Complete and submit required documentation and orders in a timely manner.
• Participate in community health initiatives when opportunities arise to promote home health and rehabilitation services.

Patient Population / Client Focus

Your primary focus will be on adults and elderly patients requiring rehabilitation support in their homes. You will address challenges related to chronic pain, post-surgical recovery, and mobility impairments, developing tailored interventions that directly impact patient autonomy and quality of life.

Qualifications

The ideal candidate will possess:

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current physical therapy licensure in the state of Virginia, with no disciplinary actions.
• At least 2 years of experience in a physical therapy role, specifically within acute care or home care settings.
• A valid driver's license and current automobile insurance to facilitate travel between patient homes.
• Current CPR certification.
• A strong commitment to clinical excellence, superior quality of care, and integrity in practice.
• An ability to engage positively with patients and their families, fostering an atmosphere of trust and collaboration.
• Outstanding communication skills, necessary for effective interaction not only with clients but also with healthcare colleagues.

Preferred Experience

Preference will be given to candidates who demonstrate:

• Experience with home health therapy practices and familiarity with OASIS documentation.
• Additional certifications related to specialty areas of physical therapy.
• Proficiency in utilizing electronic health record systems to document patient care.
• A history of leadership roles in clinical settings

Compensation and Benefits

This full-time position offers an annual salary between $100,000 and $110,000, along with comprehensive benefits including:
