Work Environment

This position is onsite within a home care facility, allowing you to engage directly with patients in their own environments. Key Responsibilities

• Deliver comprehensive physical therapy services to an average of 7 patients daily, utilizing a variety of therapeutic modalities tailored to each individual's recovery journey.
• Conduct thorough evaluations to assess patient mobility, strength, and overall physical health, creating customized rehabilitation plans that address their unique challenges and goals.
• Utilize advanced therapeutic techniques, including manual therapy, strength training, and specialized technology to facilitate patient recovery and enhance functional outcomes.
• Educate patients and their families on treatment options, self-management strategies, and preventative measures to optimize long-term wellness.
• Document patient progress meticulously, utilizing our electronic health records (EHR) system to ensure compliance with regulatory standards and facilitate seamless communication within the team.
• Collaborate with physicians, occupational therapists, and other healthcare providers to establish a comprehensive treatment approach that addresses complex patient needs.

Patient Population / Client Focus

Our clinic primarily focuses on orthopedic rehabilitation, including post-surgery recovery, sports-related injuries, chronic pain management, and age-related mobility issues. Qualifications

• Current licensure to practice as a Physical Therapist in New York state.
• Bachelor's degree in Physical Therapy or a related field (Master's or Doctorate preferred).

Compensation and Benefits
